Tournament / Event Forms to Download

The Forms below are available to be downloaded from the Welsh Hockey Web Site.

Each Form is in Microsoft Word Format (.DOC)

To download any (.DOC) form you will normally require Microsoft Word to be installed on your PC.   If you do not have Microsoft Word then a Microsoft Word Viewer is freely available from the Web, click here to download Microsoft Word Viewer.   The Word Viewer will allow you to view the form and print the form, it will not allow you to edit it.

Remember you will have to install the above viewer after downloading.

Each Form has been subject to design criteria to ensure consistency in presentation and format.   It is recommended that you do not change the document format.   There is no reason however why any user should not replace the Welsh Hockey Logo and Title with their own details.

The Forms below have been used regularly for a number of years in administering Tournaments and Events, both International and Domestic.

Using Microsoft Word the Team Sheet can be used to record the details of each team, once confirmed the information can then be copied (pasted) into the relevant Match Sheets.
